Meet for a Planning Interview
Our intake coordinator will meet with you and your family or guardian to discuss your needs and goals. You will receive a decision within 7 business days.

Develop Your Personal Care Plan
A dedicated service coordinator will create an individualized plan based on your Person-Centered Service Plan (PCSP), goals, and waiver eligibility.
